Specialty Care Manager
Provide intake and ongoing care management services to individuals living in Monroe, Finger Lakes or Southern Tier Regional Counties who have been diagnosed with one or more chronic illnesses. Effectively support ongoing and collaborative communication among all program staff and ensure a trauma informed care environment.

Essential Duties and Responsibilities

Serve as contact for referrals and conduct necessary screening and follow-up to advance intake.

Provide client-guided care management services, maintaining accurate documentation of all services rendered.

Complete comprehensive assessments, reviews, crisis plans, and care plans using a person-centered approach, meeting DOH and Health Home contractor standards.

Demonstrate cultural competency, respect, and understanding toward clients.

Recognize and embrace diversity, supporting strengths of each individual served.

Provide meaningful core service for each client monthly to ensure care plan goals are met.

Maintain accessibility during workday through a team approach.

Assist clients with acquisition and maintenance of public benefits (Medicaid, SNAP, Social Security, etc.).

Coordinate linkage/referrals for medical care, mental health, social, recreational, and housing/support needs.

Remain current in the field, implementing best practice service models.

Make independent decisions regarding immediate risk factors; provide crisis intervention and follow-up as needed.

Demonstrate flexibility to be available in homes/communities, including outside normal business hours as needed.

Provide on-call availability via cell phone to Health Home program as needed.

Advocate for clients and facilitate development of a Care Team to carry out individualized strength-based Plan of Care.

Participate in staff meetings and other Care Coordination department activities.

Ensure compliance with all applicable local, state, federal, and Health Home Network regulations.

Participate in mandatory ongoing training and professional development.

Represent Catholic Charities Family and Community Services on appropriate boards and committees as assigned.

Other duties as assigned.

Qualifications

Bachelor’s degree in Human Services or related field.

At least two years of paid experience working with individuals with disabilities or chronic illness.

Valid NY State Driver’s License with acceptable driver’s record.

Bilingual (English/Spanish) required for some positions.

Employees are required to follow appropriate supervisory direction and perform other related duties as required.
