As a Social Work Case Manager (SWCM), you are responsible for contact with program patients and assessing any barriers that may be present for a successful initiation and completion of therapy. The SWCM will have an active role with each patient from the first point of patient program enrollment and throughout treatment to facilitate a timely and successful patient experience. This role may be eligible for a hybrid schedule after 6 months (or sooner once proficiency in role is demonstrated). Review our Benefits & Perks below to learn more!
A typical day in the life of a Social Work Case Manager will include the following:
The responsibilities include, but are not limited to the following:
Serve as a consistent resource throughout the continuum of program services, which includes identifying barriers to therapy and helping the patient understand and navigate such barriers.
Serve as an educator who possesses knowledge and practical experience about oncology treatments and the strong emotions and financial implications associated with a cancer diagnosis.
Have primary goals that include:
Enable patients with information and education about:
their prescribed therapy and course of treatment
their insurance coverage benefits, anticipated out-of-pocket costs associated with their therapy and resources for financial assistance
support to help better manage the stress involved in their diagnosis.
Assess and monitor distress screenings and utilize these to triage patient needs and refer to appropriate resources
Empower the patient to participate in their personal care process by triaging the patient to support for other non-program needs such as social or other intervention programs.
Reporting of Adverse Events/ Product Complaint received in accordance with SOP and good manufacturer practices
Other duties as assigned by management

Successful candidates possess the following personal attributes:
Experience in social work with demonstrated competence in educating and managing patients with a cancer diagnosis
Knowledge about aspects of oncology including therapy options and the financial and/or psychosocial impact diagnosis has on a patient and family
Ability to travel as needed for onsite presentations to provider teams and their patients on the program services
Ability to exercise objectivity and good judgment relating to difficult and emotionally charged situations
Ability to interface effectively with patients, families, providers, management and employees while maintaining appropriate boundaries
General computer skills
Able to adapt to changing work assignments
Be service oriented and anticipate, recognize and meet needs of others
Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, and physicians
Ability to multitask effectively
Ability to recognize emotions and their effects
Sureness about self-worth and capabilities

Requirements:
2-3 years of social work experience strongly preferred
Bachelor’s degree in social work, arts and sciences, social work, public health administration or a related field strongly preferred
Experience with oncology or complex illness case management preferred
Possess excellent assessment skills
Experience and demonstrated skill in working with a transdisciplinary team
This is an office-based position, the ability to sit for extended periods of time is necessary
We are located in Jeffersonville, IN. You must be willing to work in this location; PharmaCord does reimburse for tolls if applicable, at the frequent user rate. This rate is applied after 40 trips per month (valued at $678.60 per year)
Physical Demands & Work Environment:
While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to sit for long periods of time, use hands to type, handle or feel; and reach with hands and arms. Must be able to type 35 WPM with 97% accuracy.
Although very minimal, flexibility to travel as needed is preferred.
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, etc

This role may be eligible for a hybrid schedule (50% in office, 50% remote). To qualify for a hybrid schedule, you are required to work a minimum of 6 months on-site at our headquarters location. You must show you've successfully completed training, are in good performance and attendance standing, and are consistently meeting KPI's as outlined within the program business rules.
Multiple shift options between 8 AM and 8 PM Monday - Friday
