Overview
Works under the supervision of the Manager/Director and is responsible for working in collaboration with the healthcare team to coordinate the care and service to patients across the continuum of care. Promotes effective utilization and monitoring of healthcare resources and assumes a leadership role with the interdisciplinary team to achieve optimal quality, clinical, and resource outcomes. Works under the direction of the leadership team to provide a comprehensive range of services or clinical care in accordance with standards of excellence established by Riverside Health system and facility goals and strategic plans.

Responsibilities
Build trusting, collaborative relationships with staff, peers, physicians, and other disciplines/ancillary services. Complete the problem-solving cycle from identification to resolution to maximize opportunities or minimize problems. Demonstrate commitment to co-workers.
Complete discharge planning processes aligned with patient readiness. Conduct admission and readmission assessments at appropriate times.
Perform disease-specific interventions based on evidence-based criteria to improve outcomes. Communicate discharge plans and any changes to patients, families, and healthcare professionals.
Seek to attain knowledge and competency reflecting current clinical practices, guidelines, and measures, promoting the mission, vision, and values of RHS.
Provide timely assessment and evaluation of individualized, interdisciplinary care plans communicated across the continuum. Apply standard work for improved quality and safe transitions.
Assist co-workers, physicians, and team members in patient care, addressing customer needs regardless of assignment. Strive to meet or exceed the needs of patients, demonstrating behaviors aligned with the "RESPECT" program.
Qualifications
Education: Bachelor's Degree in Sociology/Social Work (Required); Master's Degree preferred.
Experience: 3-4 years of social work experience in a clinical setting (Required).
Licenses and Certifications: Licensed Clinical Social Worker (LCSW) in Virginia preferred upon hire.
