Overview LSF envisions a world where children are safe, families are strong, and communities are vibrant. LSF is seeking a talented SNAP Case Manager who will work directly with the SNAP program's fatherhood initiative. The case manager will work with fathers who are parents of youth ages 12-17, providing case management services to the fathers, and will also run SNAP groups for youth under 12 (ages 6-11).
Purpose and Impact: Responsible for the day-to-day case management and system navigation of SNAP®.
Staff in the fatherhood initiative will act as a Case Manager for fathers of youth ages 12-17, primarily working with adults who are parents of youth 12-17. An additional responsibility is running SNAP groups for youth under 12.
Responsibilities Completes staff training as required by SNAP® and the agency.
Completes intake assessments for SNAP®, including all required pre- and post-assessments.
Responsible for the day-to-day case management and system navigation of SNAP®.
Follows up on incoming referrals and schedules clients for intake appointments.
Monitors youth and family progress through weekly compliance calls.
Facilitates SNAP® curriculum within the fidelity of the model.
Liaises with community outreach responsibilities including schools, community-based agencies, local DCF and DJJ partners, state attorneys’ offices, and police departments to enhance outreach.
Completes data reporting for intake assessments and ongoing weekly data reporting to relevant systems in a timely manner (examples: NETMIS, JJIS, Dropbox, ASEBA).
Prepares weekly materials for SNAP® groups (handouts, manuals, and other required items).
Supports SNAP® Coordinator with organization of food arrangements as needed.
Participates in pre-briefing and de-briefing meetings weekly with SNAP® Team members.
Participates in weekly consultation calls with the Florida Network.
Willingness to work nontraditional hours, including evenings when Groups are scheduled.
Prepares technology required for weekly group delivery.
Uses a strength-based perspective to help youth and families strengthen and manage the quality of their lives, engaging the family, advocating on their behalf, initiating change, helping access supports, teaching problem solving, modeling productive behaviors, and supporting youth and family milestone achievement.
Works under the supervision of the SNAP® Coordinator and participates in staff meetings as requested.
Physical Requirements Ability to sit or stand for extended periods in an office or remote work environment.
Frequent use of computer, telephone, and standard office equipment.
Must be able to lift and carry up to 20 pounds.
Ability to travel occasionally for home visits, outreach, or training.
Visual and auditory acuity to perform detailed work, including reading fine print and hearing conversations in person or over the phone.
Manual dexterity for keyboard and paperwork; may require walking, stairs, or navigating facilities during site visits or client interactions.

Education A Bachelor’s degree in Social Work or a related field.
Experience 1-3 years of experience with structured groups and parent education groups is required; community/agency liaison experience is desired.
Knowledge, Skills, Abilities Communications Skills: Excellent verbal and written communication; strong grammar and punctuation.
Computer Skills: Proficiency with Word and Excel; ability to upload videos to Dropbox and use video conferencing tools.
Analytic Ability: Knowledge of methods for data compilation and problem solving techniques.
Interpersonal Skills: Professionalism in office operations and external communications; ability to work as part of a team and with outside agencies.
Ability to service all circuit 20.
Other Demonstrates sensitivity to cultural and socioeconomic characteristics and needs of the service population.
Adherence to Florida law including statute FS 744 and Chapter 39.
Principal Accountabilities Reports to SNAP Site Coordinator;
Works independently and with other staff and outside contacts;
Ensures accurate, complete, and timely submission of all required internal and external reports;
Maintains professionalism in all LSF matters.
