Qualifications
The Care Managers main purpose is to ensure their client caseload is provided with the necessary services they need, in a coordinated and effective manner. Responsible for the caring and compassionate delivery of case management, advocacy, and other supportive services to ensure appropriate coordination of services for clients.
Qualifications Required
HS diploma or GED plus 4 years in Behavioral Health or combined Behavioral Health education and experience to meet Behavioral Health Technician qualifications.
One year case management experience strongly preferred.
Ability to create plans and summaries based on client needs.
Certification in first aid, CPR and Crisis de‑escalation (training will be offered if not possessed on hire).
Must be at least 21 years of age or older.
Possess a valid driver’s license with an acceptable driving record and reliable transportation.
Prior experience in the behavioral health setting is strongly preferred.
Demonstrated organizational abilities to maintain communication, coordination, and connections with clinical team participants.
Willingness to work evenings as needed.
Desire and capacity to maintain time‑management and organization while working in a fast‑paced, hands‑on work environment.
Must obtain and maintain a fingerprint clearance card and pass a pre‑employment drug and background check.
Have a tuberculosis screening test and provide evidence of freedom from infectious tuberculosis prior to providing services; additional TB testing or screening will be performed annually.
CPR/First Aid Certification; training will be provided if the candidate does not possess active certification.
Possess the physical, mental, and cognitive ability to perform specific clinical and/or tasks required by the position without posing a direct threat to the safety and security of patients.

Experience working in an integrated health care, behavioral health, or primary care environment.
Advanced knowledge of cultural diversity and sensitivity.
