Responsibilities Collaborate with a dynamic clinical and medical team to empower patients to care for their physical and mental health.
Conduct hands-on skills-building sessions in community, home, and school settings.
Provide support and guidance to individuals in emotion regulation, relational skills, independent living skills, and general coping skills.
Utilize a comprehensive toolbox of evidence-based practices to enhance patient well-being.
Maintain accurate and up-to-date records of patient progress and treatment plans.
Act as a compassionate and dedicated advocate for the individuals and families served.
Provide empathetic support to a diverse caseload, including a variety of ages, diagnoses, and backgrounds.

Logistics & Requirements Must be at least 21 years of age.
Must have valid driver’s license and reliable vehicle.
Must meet at least one of the following educational requirements:
Bachelor’s Degree
Associate’s Degree
60 undergraduate college credits
High school diploma/GED with four years related experience
Base Pay and Compliance Bachelor’s Degree: $20.00 - $22.00/hour
Associate’s Degree: $18.00 - $20.00/hour
Note: As a state-of-the-art healthcare organization and federal contractor, all team members are required to receive a flu shot between October and March and TB test to protect staff and patients. Exemption options exist but approval is not guaranteed.
