Cortica is looking for a Site Care Coordinator to join our growing multi-disciplinary team.
What you’ll do
Overview The Site Care Coordinator plays a crucial role in optimizing client and provider scheduling to enhance service delivery and operational efficiency. This role is a consistent point of contact for families and interfaces with site leadership across clinical, operational, and shared services (e.g., RCM, Scheduling, Enrollment) to maintain a cohesive site culture and guide families through their Cortica journey. The position requires on-site presence to manage scheduling processes and address concerns promptly.
Responsibilities Support scheduling optimization to maximize provider capacity while aligning with client needs and care plans.
Act as the primary contact for families, coordinating with clinical, operational, and shared services to ensure cohesive service delivery.
Facilitate communication between families and teams to ensure seamless coordination of care.
Manage scheduling dynamics, including cancellations and rescheduling, and implement scheduling practices aligned with client needs and provider capacity.
What will you do? Patient Care Navigation Guide patients through care pathways efficiently, monitor progress, and proactively identify and address delays or barriers.
Serve as the point of contact for families and streamline communication throughout their Cortica journey.
Keep families informed about wait times for services.
Scheduling Optimization Develop scheduling plans that adhere to best practices, maximizing provider utilization while meeting client needs.
Collaborate with clinical teams to integrate scheduling strategies into broader care plans.
Provide scheduling support for contract scheduling processes, particularly for Schools-related contracts, and assist with scheduling for Value-Based Care (VBC) clients.
Communication and Collaboration Work with on-site leadership to address and resolve client scheduling concerns promptly and professionally.
Collaborate with clinical leadership to ensure timely case assignment and management.
Respond to client inquiries and requests with high levels of service.
Technology Utilization and Best Practices Utilize scheduling tools such as Axon, Smart Scheduling, and Salesforce to improve efficiency.
Monitor KPIs to assess scheduling success and inform data-driven improvements.
Follow established scheduling protocols and best practices to meet the needs of clients and providers.
Qualifications 4+ years of scheduling experience in ABA services or related field.
2+ years in a client services related role.
Strong problem-solving skills.
Compensation and Benefits $2,000 continuing education stipend.
Competitive compensation: $26.01/hr to $32.51/hr depending on experience.
