Service Care Coordinator – Coastal Family Health Center, Astoria, OR
Coastal Family Health Center is a leading community health organization with 40+ clinics across Washington and Oregon. We provide a full range of medical, dental, pharmacy, orthodontia, nutritional counseling, autism screening, and behavioral health services and extend support for shelter, energy, weatherization, HIV and AIDS counseling, home visits, and mobile medical/dental clinics.

Position Highlights
$24.01–$29.42 DOE with potential higher pay for highly experienced candidates

What You’ll Do
Execute incoming calls, assess client needs, and identify appropriate services.
Collect documentation to support program eligibility.
Assist clients with scheduling appointments or classes.
Process referrals for medical, dental, and behavioral health services.
Monitor and document active referrals and client participation.
Assist with completion of applications and forms required by the program(s).
Respond to basic questions from patients.
Provide follow‑up contact with patients regarding services and referrals recommended; encourage participation and retention.
Adhere to productivity goals including number of patients, encounters, appointments scheduled, and classes facilitated.
Provide program administrative support such as data entry, client file management, report development, and contract compliance.
Coordinate and promote YVFWC programs; recruit clients for dental, medical, social services or education classes; reserve conference rooms, obtain supplies, organize snacks for classes.
Present program information in internal and external community meetings.
Perform other duties as assigned.

Qualifications
High School Diploma or GED.
One year of administrative support and customer service experience.
Two years’ administrative support experience in a social services or healthcare setting is preferred.
Applicable license or certification; some positions may require program specific certification.
Valid Driver’s License and proof of automobile liability insurance coverage.
Bilingual (English/Spanish) preferred; must demonstrate language proficiency level 9 for differential pay.
Strong organizational, written, and verbal communication skills; ability to prioritize multiple tasks simultaneously.
