Overview RN / Registered Nurse Case Manager - Hospice
Office Location: 744 Horizon Ct #110, Grand Junction, CO 81506
Coverage area: Delta, Garfield, Mesa
Schedule: Monday - Friday 8:00 AM - 5:00 PM
Location: US-CO-GRAND JUNCTION
Responsibilities Assess/monitor physical, emotional, and psychological needs of patients
Create hospice care plans that align with the patient\'s wishes and goals
Direct nursing care: administering medications, treatments, and interventions
Provide pain and symptom management
Educate and support the patient\'s family and caregivers
Collaborate with interdisciplinary team
Maintain accurate and timely documentation
Participate in on-call rotation as required by local branch

Qualifications Graduate of an accredited nursing school with current licensure to practice in state of operation
One year nursing experience required, 3+ years preferred
Experience in hospice or a similar setting preferred
Knowledge of the hospice philosophy of care
Commitment to clinical and documentation excellence
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

Salary Range: USD $80,000.00 - $85,000.00 / Year
