Job Overview As an RN/Nurse Case Manager at Black Hills Special Services Cooperative (BHSSC), you will coordinate medical care for individuals with developmental disabilities, manage caseloads, monitor medications, and collaborate with direct support staff and healthcare providers to ensure timely, person-centered care. This part‑time position (20–29.75 hours weekly) pays $33.00–$36.00 per hour based on experience.
Responsibilities Maintain awareness of the health and medical goals and needs of each person supported.
Provide support to meet routine and emergency medical and health needs, adapting care to fit the individual’s circumstances.
Respond promptly to questions and communications from direct support staff and other stakeholders regarding healthcare information.
Ensure adequate supply of medication, enteral nutrition, and medical supplies are always available for each person supported.
Monitor follow‑up on recommendations and orders from medical staff and document outcomes.
Use the online documentation system platform as required by DD Division.
Maintain current medical files, as dictated by licensing standards, and keep documentation current, including immunizations.
Complete health‑care reports on therapeutic services for people supported.
Perform other duties as assigned.
Qualifications Current valid South Dakota nursing license.
Current valid driver’s license.
Current CPR certification.
Ability to pass criminal records check, background check, and pre‑employment drug screening.
Organizational and communication skills with the ability to self‑motivate.
