Position Details The corporate office is located in Southfield, MI and the RN will provide support throughout our service area within the Tri-County area (Oakland, Macomb & Wayne). An RN with ICU, PICC Line, wound care, and infusion experience is greatly preferred.
A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence, in accordance with physician orders and plan of care under the direction of the Clinical Manager/Nursing Supervisor.
Qualifications Graduate of an approved school of professional nursing and currently licensed in the state of Michigan.
One year of nursing experience.
Kinnser - EMR experience preferred.
Home Healthcare experience preferred.
Experience in wound vac, PICC line and mediport care, preferred.
Pediatric experience, desired.
Responsibilities Provide services in accordance with the plan of care and perform initial evaluation and ongoing reassessments of patient nursing needs.
Initiate and revise the plan of care; provide services requiring specialized nursing skills.
Initiate preventive and rehabilitative nursing procedures; prepare clinical and progress notes; coordinate services.
Inform personnel of changes in patient condition; counsel the patient and family in meeting nursing needs; participate in in-service programs.
Adhere to agency policies and procedures; process orders, notify physicians of changes, and complete certification/recertification as required.
Determine nursing needs per patient and refer to Physical Therapy, Speech Language Pathology, Occupational Therapy, and Medical Social Work as appropriate.
Supervise and teach other nursing personnel; conduct patient care conferences; participate in peer review and performance improvement activities.
Participate in utilization review of medical records as assigned; provide total patient care as needed; take on-call duty nights, weekends and holidays as assigned.
Complete OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant changes in condition within agency-defined time frames; appropriately utilize ICD-10 codes.
Working Environment Works indoors in the agency office and patient homes and travels to/from patient homes.
