We currently have a position available for an experienced Home Health Care Registered Nurse. A RN with Mediport, PICC Line, Wound Care, and Infusion experience is greatly preferred.
The corporate office is located in Southfield, MI, and the individual will provide RN support throughout our service area within the Tri-County area (Oakland, Macomb & Wayne).
A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their residence, in accordance with physician orders and plan of care under the supervision of the Clinical Manager/Nursing Supervisor.
Qualifications: Graduate of an approved school of professional nursing and currently licensed in Michigan.
One year nursing experience.
Kinnser - EMR experience preferred.
Home Healthcare experience preferred.
Experience in Wound Vac, PICC line, and Mediport care, preferred.
Pediatric experience, desired.
Responsibilities: Provides services per the plan of care.
Makes initial and regular reevaluation visits.
Initiates and revises the plan of care.
Provides services requiring specialized nursing skills.
Prepares clinical notes and summaries.
Coordinates services and informs personnel of patient condition changes.
Counsels patients and families.
Participates in in-service programs and adheres to agency policies.
Processes orders, notifies physicians, and completes certification/recertification and discharge summaries.
Determines nursing needs and refers to specialists as needed.
Supervises and teaches other nursing personnel.
Conducts patient care conferences.
Participates in peer review, performance improvement, and utilization review.
Provides total patient care as needed and takes on-call duty as assigned.
Completes assessments and utilizes ICD-10 codes appropriately.
Working Environment: Indoor work at agency office and patient homes, with travel involved.
