We seek an experienced Home Health Care Registered Nurse. Experience with Mediport, PICC Line, Wound Care, and Infusion is highly preferred.
The corporate office is in Southfield, MI, and the individual will provide RN support throughout the Tri-County area (Oakland, Macomb & Wayne).
A Registered Nurse administers skilled nursing care to patients in their homes, following physician orders and the plan of care under supervision of the Clinical Manager/Nursing Supervisor.
Qualifications: Graduate of an approved nursing program and current Michigan license.
At least one year of nursing experience.
Kinnser EMR experience preferred.
Home Healthcare experience preferred.
Experience in Wound Vac, PICC line, and Mediport care preferred.
Pediatric experience is a plus.
Responsibilities: Provide services according to the plan of care.
Conduct initial and ongoing patient evaluations.
Develop and revise the plan of care.
Perform specialized nursing procedures.
Coordinate and document patient care.
Communicate patient condition changes to the team.
Educate patients and families.
Participate in in-service training and quality improvement activities.
Follow agency policies and procedures.
Process orders, notify physicians of changes, and complete documentation.
Refer patients to other healthcare professionals as needed.
Supervise and educate other nursing staff.
Participate in patient care conferences and peer reviews.
Assist with on-call duties during nights, weekends, and holidays.
Complete required assessments and documentation within specified timeframes.
Work Environment: Indoor office and patient homes, with travel within the service area.
