Base pay range: $71,100 – $97,800 per year
Responsibilities
Perform full scope of care coordination activities for members needing coordination.
Serve as single point of contact for care coordination.
Assess and deliver resources to members/families for optimal wellness.
Collaborate with providers and community services to promote quality outcomes.
Coordinate delivery of needed services/supports for Physical Health and Social Determinant of Health benefits.
Qualifications
Must reside in Ohio.
Registered Nurse (RN) in Ohio without restrictions.
Minimum two years of clinical experience, including Maternity, Labor & Delivery, Mother Baby.
Excellent communication and interpersonal skills.
Ability to work with minimal supervision within role scope.
Valid driver’s license; proof of personal vehicle liability insurance with 100,000/300,000/100,000 limits.
High‑speed DSL or cable modem required for home office; satellite and wireless not allowed.
Preferred Qualifications
Case Management Certification (CCM).
Experience with Medicare, Medicaid or dual‑eligible populations.
Bilingual (Haitian Creole, Spanish, Somali or other).
