RN Clinical Manager - Palliative Care Advanced Illness Management
Overview: Expand Care. Lead with Compassion. Transform Lives. We are seeking a Clinical Manager to join our Advanced Illness Management (AIM) team. This role reports directly to the Director of AIM and is responsible for coordinating and managing the day-to-day business affairs and clinical activities of the AIM and Palliative Care department. The Clinical Manager ensures excellence in clinical care delivery, patient-centered service, and team leadership.
Key Responsibilities
Coordinate and oversee daily clinical activities within the AIM program, including managing Clinical Coordinators, Nurse Practitioners (NPs), and Registered Nurses (RNs).
Serve as the primary contact for patient and partner calls, ensuring appropriate and timely follow-up.
Manage referral processes, admission information, and insurance verification to support timely patient admission.
Ensure efficient scheduling for Nurse Practitioners, considering geographic and cost factors.
Monitor visit frequency compliance, risk scoring, and hospitalization tracking to support quality outcomes.
Coordinate orders for labs, imaging, and DME; track and provide results to NPs and PCPs as appropriate.
Maintain EMR documentation, medication reconciliation, and provider communications.
Oversee credentialing documentation and maintain credentialing logs.
Direct and maintain clinical record systems to ensure compliance with state, federal, and company policies.
Support interdisciplinary team meetings and coordinate agendas.
Manage referrals to Home Health and Hospice, ensuring smooth care transitions.
Foster strong professional relationships with referral sources, patients, families, and community partners.
Support quality assurance initiatives, performance improvement, and marketing strategies.
Participate in staff recruitment, onboarding, training, and evaluations.
Qualifications
Registered Nurse with Home Health, Hospice, or Palliative Care experience preferred.
Bachelor’s degree in Nursing, Business Administration, or related field required (Master’s preferred).
Minimum of 2 years of healthcare administration or operations management experience required.
Certification in Hospice and Palliative Care preferred.
Strong leadership, communication, organizational, and problem-solving skills.
Proficiency in Microsoft Office Suite; EMR experience required.
Reliable transportation, valid driver’s license, and insurance required.
