Good Samaritan Home Health is seeking a compassionate and skilled Registered Nurse (RN) Case Manager to join our dedicated Home Health team. In this role, you’ll provide high-quality, patient-centered care to individuals in the comfort of their homes, while coordinating services to ensure patients and families receive the support they need.

What You’ll Do
Provide skilled nursing care, assessments, education, and support to patients and families.
Serve as the primary case manager, coordinating care with physicians, therapists, aides, and other team members.
Complete accurate and timely OASIS documentation, admissions, recertifications, and discharges.
Monitor patient conditions, update care plans, and communicate changes to the interdisciplinary team.
Educate patients and caregivers on disease management, medications, and strategies to promote independence.
Qualifications
Minimum 1 year of nursing experience (home health or hospice preferred).
Medicare/OASIS documentation experience a plus.
Current CPR/BLS certification (or ability to obtain within 30 days of hire).
Strong clinical assessment, communication, and critical thinking skills.
Valid driver’s license, automobile insurance, and reliable transportation.

Shift Details: Flexible schedules are available (40hrs, 32hrs, 24hrs, or scheduled PRN)
