CenterWell Home Health Home Health RN Case Manager
Base pay: $70,500.00 - $96,900.00 per year.
Pay per visit/unit: $45.00 - $63.00.
Scheduled weekly hours: 40.
Responsibilities
Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
Administer ongoing care and case management for each patient, with follow‑up as directed by the Clinical Manager.
Confer with the physician to develop the initial plan of treatment based on physician orders and initial patient assessment; provide hands‑on care, manage and evaluate the care plan, and teach the patient under the Clinical Manager’s supervision.
Coordinate appropriate care with physical therapists, occupational therapists, home health aides, and external providers.
Report patient care, condition, and progress to the patient’s physician and Clinical Manager continuously.
Implement the patient care plan with the patient and family to achieve optimal resolution of needs and problems.
Coordinate, supervise, and oversee the work of Home Health Aides and Personal Care Workers; provide written personal care instructions and monitor documentation.
Discharge patients after consultation with the physician and Clinical Manager, preparing needed clinical documentation.
Prepare all medical documentation, including case conferences, medication orders, re‑certifications, progress updates, and care plan changes; update patient records and confer with other disciplines to ensure optimum care.
Required Experience and Skills
Diploma, Associate, or Bachelor Degree in Nursing.
Minimum of one year of nursing experience preferred.
Strong experience in medication surgery, ICU, ER, or acute settings.
Experience in Home Health is a plus.
Current unrestricted RN licensure.
Current CPR certification.
Strong organizational and communication skills.
Valid driver’s license, auto insurance, and reliable transportation.
