Overview RN Case Manager Home Health role at CenterWell Home Health. Provide personalized, one-on-one nursing care at patients\' homes. Develop and manage care plans in collaboration with physicians and clinicians to support recovery and help patients regain independence.
Responsibilities Provide admission, case management, and follow-up skilled nursing visits for home health patients.
Administer ongoing care and case management for each patient, with follow-up as directed by the Clinical Manager.
Confer with physician to develop the initial plan of treatment based on orders and initial patient assessment. Provide hands-on care, management and evaluation of the care plan, and education of the patient in accordance with physician orders, under the Clinical Manager\'s supervision. Revise plan in consultation with physician based on ongoing assessments and policy/regulation.
Coordinate appropriate care with Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
Report patient care, condition, and progress to the patient\'s physician and Clinical Manager on an ongoing basis.
Implement the patient care plan with patient and family to help achieve optimal resolution of needs and problems.
Coordinate, supervise, and oversee the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers; provide written care plans reflecting the current plan of care. Ensure documentation by home health aides and personal care workers is completed as part of supervision.
Discharge patients after consultation with the physician and Clinical Manager; prepare and complete necessary clinical documentation.
Prepare medical documentation for all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports and summaries; coordinate with other health care disciplines to provide optimum patient care.
Qualifications Diploma, Associate, or Bachelor Degree in Nursing
Minimum of one year of nursing experience preferred
Strong med-surg, ICU, ER, and acute care experience
Home Health experience is a plus
Current and unrestricted Registered Nurse licensure
Current CPR certification
Strong organizational and communication skills
Valid driver’s license, auto insurance, and reliable transportation
Pay and Schedule Base pay range: $70,500.00/yr - $96,900.00/yr
Pay per visit: $45.00 - $63.00
Scheduled weekly hours: 40
