We are looking for an experienced, dynamic RN Case Manager to join our team.
Position Summary
As a Case Manager , you will be responsible for the coordination of care with the patient, family, and other members of the team. Our Case Managers work as RN/LPN teams so we need someone with great supervisory and organization skills.
This is an opportunity to work for a locally based company that is focused on providing high quality care in a comfortable and safe environment. We are looking for nurses who are looking to make a difference in the lives of our patients and their families.
Knowledge, Skills And Abilities
Must be comfortable with driving.
Must be comfortable using EMR
Must have an energetic and positive demeanor
Must be able to remain calm under patient pressure.
Education & Experience
Must be currently licensed as an RN in Georgia
Previous experience in hospice, preferably working with RN/LPN teams
