Job Summary Responsible for providing comprehensive assessment, planning, implementation and evaluation of individual patient needs. Collaborates with Physicians, patient/family, nursing, utilization review and other members of the healthcare team to ensure patient management aligns with patient needs using resources to achieve quality, clinical and cost-effective outcomes. Coordinates a team approach to facilitate achievement of expected patient outcomes with appropriate transitions to the next level of care. Manages length of stay, regulatory compliance, and participates in interdisciplinary rounds on the assigned unit. Collaborates with community providers to coordinate post-hospitalization care. This position serves patients across neonate, infant, child, adolescent, adult and geriatric populations. Performs clinical duties in accordance with population-specific guidelines and National Patient Safety Guidelines. Provides cross coverage for RN Case Management as required across all settings in the care continuum, including weekend rotation as needed.
Minimum Job Qualifications Licensure: Licensed to practice as an RN in Georgia.
Educational Requirements: Associate degree; graduate of an accredited school of nursing.
Minimum Experience: 3–5 years of direct patient care and/or case management; financial and discharge planning experience.

Preferred Job Qualifications CCM (Case Management Certification) or ACM (American Case Management Certification) preferred.
Bachelor's Degree preferred.

Essential Tasks and Responsibilities Monitor patients on assigned units to ensure appropriate use of resources and manage length of stay based on DRG/admitting diagnosis.
Communicate with physician, patient/family, and other disciplines regarding expected length of stay and progress toward discharge.
Coordinate and oversee patient care to ensure timely interventions; identify delays and escalate as needed.
Assess and facilitate discharge planning; ensure post-acute needs are arranged and communicated to the care team; reassess plan regularly for timely and safe discharge.
Maintain knowledge of reimbursement methodologies and coverage guidelines; coordinate with physician/patient/family to ensure services are covered.
Coordinate with Utilization Review Nurse daily to ensure appropriate status and level of care; facilitate changes as needed.
Serve as liaison to patients, families, physicians, nursing staff and other disciplines to optimize discharge planning; provide leadership on unit for discharge planning and community resources related to patient care and outcomes.
Support a customer-service oriented environment; ensure proper hand-off communication during transitions between units.
Maintain positive attitude and adaptability to change; contribute to ongoing improvements in daily workflow.
Maintain knowledge of community resources, regulatory requirements and third-party payer requirements; complete required paperwork.
Demonstrate a sense of urgency while maintaining financial stewardship.
Work all scheduled shifts, including weekend rotation and remote coverage as needed.
