The RN Case Manager provides leadership abilities with strengths in the following areas:
Understanding, identifying, and assisting in the coordination of the needs of the terminally ill and their families with a strong commitment to the hospice concept.
Ability to understand and promote effective interaction between the IDT, and other hospice members, nursing homes, hospitals, and community members.
Effective verbal, nonverbal, and written communication skills.
Supervision of CNAs, LPNs and other paraprofessionals providing care the patient and family.
Physical and emotional health and personal qualities which enable him/her to work effectively with patients, families, and staff, including satisfactory adjustment to significant personal losses.
A strong desire for continued personal and professional growth and development.
The ability to perform duties independently.
The ability to work non-judgmentally with patients and families of many cultural, religious, and socioeconomic background and variety of lifestyles.
Requirements include:
Graduate of an accredited school of professional nursing.
Current licensing as a Registered Nurse in Colorado.
BSN Preferred.
At least one year of professional experience in the delivery of nursing services, two to five years preferred.
Willingness and ability to travel within the hospice service area.
Ability to lift pounds and independently perform personal care on a patient who weighs up to two hundred and fifty pounds.
