Position Summary
Responsible for assessing transitional care needs, coordinating care across the continuum, and engaging with patient and family to assure care needs are met. Serves as a key resource for patients and consults with the care team regarding patient's psychosocial and resource needs. In conjunction with the patient and physician, will assess, coordinate, and implement a timely, safe patient discharge plan to the next appropriate level of care. Overall, the role integrates and coordinates the patients transitional care plan into their individualized discharge plans based on needs and resources available.

Responsibilities

Assess transitional care needs.

Coordinate care across the continuum.

Engage with patient and family to assure care needs are met.

Consult with care team members about psychosocial and resource needs.

Collaborate with patient, physician to develop discharge plan.

Integrate transitional care plan into individualized discharge plans.

Minimum Required Qualifications

GA/compact RN license.

AHA or ARC BLS.

1+ year of experience in healthcare in the acute care setting, related field or skilled care environment or community or educational internship in care coordination.

Epic experience.

Preferred Qualifications

Background in medical social work in an acute care setting.

Assignment Details

Length of assignment: 13 weeks.

Shift / Hours per week: Fri; Sat, Sun, Mon: 7am- 5:30pm with holiday rotation.

Systems used: Epic.

Start date: 12/22/2025.
