Position Highlights

Full‑time, Monday‑Friday, 8:00‑17:00

Location: Winston Salem Campus, Wake Forest Baptist Medical Center

Pay Range: $37.50 – $56.25 per hour

Qualifications

BSN required; MSN preferred

Registered Nurse licensure from the North Carolina Board of Nursing

Minimum 2 years of relevant clinical experience; previous case management experience preferred

Basic Cardiac Life Support (BCLS) required

Case Management certification highly recommended within 2 years of employment

Job Responsibilities

Identify patients suitable for case management based on discharge screening and need assessment

Assess all relevant data, interview patient/family, and perform objective evaluation of patient needs

Develop and implement a cost‑efficient Plan of Care with nursing to enhance patient outcomes

Collaborate with multidisciplinary teams, caregivers, and community providers to execute the Plan of Care

Coordinate discharge planning, ensuring completion of all related documentation and services

Evaluate and monitor effectiveness of discharge planning, adjusting the Plan of Care as needed

Document case management actions in the Electronic Medical Record and communicate essential information to providers and payors

Participate in multidisciplinary rounds to communicate discharge planning and drive patient progression through the system

Establish partnerships with physicians to achieve institutional goals and enhance patient care

Identify system issues and suggest interventions to eliminate duplication and document significant variances

Identify trends, process barriers, and develop plans to move patients effectively through the continuum of care

Utilize quality improvement principles in all aspects of performance

Maintain working knowledge of payer and reimbursement practices impacting the plan of care

Guide patients and families through post‑discharge care options
