Base pay: $68,224.00/yr - $104,811.20/yr.
Additional Information About The Role
Location: Alton Memorial Hospital, One Memorial Dr, Alton, IL 62002.
This position is onsite.
$2,500 sign‑on bonus for external candidates.
Full‑time; benefits eligible.
Monday – Friday, days, generally 8:00 am – 4:30 pm (some flexibility as needed).
No weekends, holidays, or on‑call requirements at this time.
Preferred Requirements
At least 5 years of hospital‑based RN experience.
Prior case management experience.
Proficient in EPIC.
Overview
Since 1937, Alton Memorial Hospital has cared for residents in Alton, Illinois, and the surrounding communities in a five‑county area. It is a full‑service acute care hospital that offers inpatient and outpatient services, including surgery, medical imaging, heart services, cancer care, rehabilitation, 24‑hour emergency care, ambulance services, and more.
Role Purpose
Coordinates, negotiates, procures, and manages the care of patients to facilitate quality cost‑effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues. Provides consultation regarding utilization management and performance improvement programs.
Responsibilities
Assesses, coordinates, and evaluates each patient's plan of care.
Establishes, continuously evaluates, and documents an individualized discharge plan.
Ensures clinical documentation and cost‑effective use of hospital resources.
Minimum Requirements
Education: Nursing Diploma/Associate's or Bachelor’s Degree in Nursing.
Experience: No prior supervisory experience required.
Licenses & Certifications: Registered Nurse (RN).
Preferred Requirements
Education: Bachelor’s Degree in Nursing.
Experience: 2–5 years of related experience.
