The Registered Nurse Case Manager provides a wide range of nursing care to patients in the home care setting. The RN Case Manager assesses, develops, implements, and evaluates home care needs.

This position may serve Mahoning, Trumbull & Columbiana counties. Looking for full-time and part-time candidates.

Responsibilities

Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients.

Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient's needs.

Manage emergency situations in a prompt, precise, and professional manner.

Complete medication administration and treatment needs for all patients assigned.

Provide nursing care in accordance with applicable regulations and company policies.

Prepare and maintain accurate and complete clinical and progress notes on a timely basis.

Provide supervision and oversight to patient care.

Utilize the Electronic Visit Verification system.

Prepare physician orders as applicable.

Participate in continued education programs, patient care conferences, and staff meetings as scheduled.

Perform nursing assessments (initial and ongoing).

Maintain excellent assessment skills and clinical knowledge and expertise.

Notify the physician and branch management of significant changes in patient status and obtain changes in orders as necessary.

Assume responsibility for ongoing interdisciplinary assessment of the patient.

Participate in branch on-call rotation as assigned.

Follow organizational policies and procedures regarding infection control, disposal of hazardous waste, and use of protective devices as evidenced by documentation.

Perform other job-related tasks and duties as assigned.

Qualifications

Education/Experience Desired: Nursing supervisory/leadership experience preferred; Six months’ experience in home care preferred.

License Requirements: Active RN state license, Valid driver’s license, Current CPR certification.

Skill Requirements

Ability to work flexible hours as required to meet patient needs.

Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the patient.

Able to work independently.

Strong commitment to the organization’s goals, mission, and philosophy.

Understanding of home health concepts and the RN’s role as part of a full discipline healthcare team.
