RN Care Manager – PACE Program

Full Time | Salary: $85,000 – $95,000
Field visits and occasional on-call required
A well-established PACE (Program of All-Inclusive Care for the Elderly) organization is seeking an experienced and mission-driven RN Care Manager to join its interdisciplinary team. This role is ideal for a nurse who thrives in a community-based care model and enjoys working closely with older adults across clinic, center, home, and facility settings.
The RN Care Manager is responsible for assessing participants’ care needs, providing nursing interventions, evaluating outcomes, and collaborating closely with the interdisciplinary team (IDT) to develop comprehensive, person-centered care plans. This role includes field visits, clinic-based care, and participation in the on-call rotation to support participant needs.
Provide high-quality clinical care as a member of the PACE interdisciplinary team
Deliver nursing care in the center, clinic, contracted facilities, and participant homes based on individualized care plans
Participate in the 24/7 on-call triage rotation
Assess, plan, and coordinate home care services; provide nursing input to the IDT for home care interventions
Monitor acute and chronic care needs, coordinating care to support aging in place or optimal facility placement
Follow up on specialist visits, gather documentation, and ensure timely communication with providers
Reconcile medication administration records (MARs) monthly for assigned participants
Notify participants of normal test results and ensure timely documentation
Complete nursing assessments and implement nursing-related care plan interventions
Educate participants, caregivers, and families on self-care, medications, safety, and health promotion
Review and update goals and care approaches with participants, caregivers, and the IDT
Collaborate with IDT members and participate in all team meetings
Assist team members in understanding participants’ functional and nursing needs
Perform responsibilities of Home Care Coordinator or other licensed roles as needed
Participate in quality improvement projects, staff meetings, and training
Evaluate competence of CNAs and Home Care Aides; delegate tasks as appropriate
Maintain confidentiality and follow all PACE, OSHA, and infection control protocols
RN licensure required
Bachelor of Science in Nursing (BSN) preferred
BLS certification (or obtained within 90 days of hire)
Minimum 1 year of RN experience required
Experience working with a frail or elderly population preferred (training available if needed)
Preferred experience in home care, long-term care, or managed care
Strong clinical assessment, communication, and interdisciplinary collaboration skills
Reliable transportation, valid driver’s license, and state-required auto insurance
Medical clearance for communicable diseases and up-to-date immunizations
Successful completion of background checks, license verification, reference checks, and drug screening
