Job Summary
The Care Coordinator collaborates with the assigned clinical team to identify patients most likely to benefit from care coordination services. Responsibilities include assessing patient risk factors, managing utilization and preventive care, and ensuring continuity of care.
Responsibilities
Takes the lead in ensuring continuity and consistency of care across inpatient, emergency, and outpatient settings, including comprehensive discharge planning and follow‑up care.
Develops an effective working relationship with Patient and Family Counselors, Social Workers, and Utilization Review Nurses to engage patients and families, advocate, and problem‑solve to support functional ability.
Educates patients and families on chronic illness evidence‑based standards of care and self‑management strategies.
Identifies support needs and develops action plans, providing guidance to overcome self‑management barriers.
Facilitates patient understanding of the healthcare system and builds relationships across settings.
Ensures patient access to prescriptions, durable medical equipment (DME), and other services.
Contributes to problem solving within the team through communication, collaboration, data collection, consensus building, and evaluating outcomes.
Advocates for patients to optimize health care needs, including safety, physical, legal, and financial well‑being.
Refers patients to education and resources regarding healthcare delivery, reimbursement, prescriptions, wellness programs, community agencies, housing options, and other services.
Uses available IT resources (e.g., Phytel, Crimson) to facilitate registry reporting and maintenance of patient populations, improve disease outcome measures, and implement clinical decision support tools.
Participates in clinical outcome measurement, identifying strategies to promote population health.
Ensures patient safety by implementing policies, procedures, and standards supporting assigned duties.
Maintains regular, punctual attendance consistent with Orlando Health policies, ADA, FMLA, and other federal, state, and local standards.
Maintains compliance with all Orlando Health policies and procedures.

Demonstrates awareness of medical/legal issues, patient rights, and compliance with standards of regulatory and accrediting agencies.
Qualifications
Education/Training: Bachelor of Science in Nursing (BSN) degree.
Licensure/Certification: Current RN license in Florida; current BLS or healthcare provider certification.
Experience: Three (3) years of direct clinical experience with an emphasis on the population to be served in the assigned area.
