This position involves determining the appropriateness of hospital admission, advocating, coordinating, and facilitating the interdisciplinary plan of care to ensure effective and timely resource utilization for optimal patient outcomes. The role partners with charge nurses, social workers, physicians, and other team members to facilitate safe discharges and remove barriers to patient throughput. Applies clinical expertise and medical criteria to resource utilization, admissions, and discharge planning.
Job Description:
Education:
Required: Bachelor of Science in Nursing (BSN) or Master’s degree in Nursing
Licensure/Certification:
Required: California RN licensure
Preferred: Certification in Case Management
Experience:
Required: Two (2) years as an RN in acute care or case management
Ensure admission review completion within one working day.
Coordinate transfers with ED and Transfer Coordinators.
Perform comprehensive assessments and establish clinical milestones and goals.
Build rapport with patients and families, considering cultural factors.
Collaborate with physicians and teams to develop care plans.
Communicate care plans and changes effectively.
Gather information to evaluate care effectiveness and ensure safety, timeliness, and cost-effectiveness.
Advise staff and physicians on admission appropriateness, care levels, and documentation.
Organize and evaluate care plans, modifying as needed.
Coordinate patient transfers and resolve clinical or operational issues.
Identify opportunities for care improvement and assist in performance improvement plans.
Understand payer requirements and ensure timely communication with payers.
Document assessments and interventions thoroughly.
Maintain appropriate care levels through ongoing reviews.
Escalate unresolved issues to the Utilization Management Committee.
Educate staff on regulatory and admission standards.
Work with ED and Transfer teams to ensure evidence-based practices are followed.
Discuss discharge plans proactively with teams and patients.
Monitor patient progression and update care plans accordingly.
Ensure safe discharge planning and follow-up.
Refer complex cases to social workers or discharge planners.
Communicate post-discharge care information to providers.
Develop community resource connections for patients.
Assist patients in obtaining primary care and medications.
Ensure follow-up appointments are scheduled and understood.
Issue notices of non-coverage/denials in compliance with regulations.
Monitor clinical outcomes and intervene as needed.
Perform additional duties as assigned.
Possess strong communication, clinical, and critical thinking skills.
