Summary
The Care Coordinator supports members by completing health and psychosocial assessments, developing initial care plans, and coordinating services with an inter-disciplinary care team (ICT). The role centers on telephonic/electronic outreach and ensuring members receive timely, culturally competent support.
Key Responsibilities
Conduct health and psychosocial assessments.
Develop and document individualized care plans with the ICT.
Identify barriers to care and implement interventions.
Educate members/caregivers on resources and benefits.
Coordinate with facility and community-based care managers to ensure smooth transitions and avoid duplication of services.
Maintain accurate, timely documentation and share process improvement ideas.
Requirements
Diploma, Associate’s, or Bachelor’s in healthcare-related field or equivalent experience.
RN License is required in Michigan.
3+ years in care coordination, case management, discharge planning, or related healthcare role.
Strong communication, organization, and documentation skills.
Knowledge of healthcare regulations and community resources.
