We are seeking a full-time Respite Care Coordinator to join our Team at our DuPage office.

Salary: The average starting salary for this position will fall in the range of $48,000 and $51,000 annually. Where candidates fall in this range will be based on skill and experience level.
Essential Job Functions / Responsibilities Conducts necessary assessments and develops collaborative, strengths-based child and family service plans.
Provides outreach and education to children and families about care coordination and eligible services.
Conducts community engagement and provides services to the child and family in home and community-based settings.
Gathers information for the assessment and treatment care plans.
Documents the care/service plans and other vital information in electronic health record after each contact and service.
Actively leads care planning process that specifies direct care resources to meet physical and psychosocial needs; prioritizes problems and establishes mutually agreed upon goals specific to the client.
Participates in participant education, including development of materials, conducting presentations or supporting other team members in such efforts.
Works with multidisciplinary professionals and community agencies to provide a continuum of coordinated care to meet the complex needs of clients.
Maintains timely and accurate client records and any necessary agency and funder required program reports.
Participates in agency and site based required meetings and trainings.
Other duties as assigned.
Knowledge, Skills and Abilities Experience working with diverse child and family populations with significant behavioral health needs; social service experience is preferred.
Excellent oral and written communication skills, including written, social media, giving community presentations and proficiency in Microsoft Office Suite and Google tools.
Punctual, reliable and dependable with the ability to work flexible hours (some evenings and weekends).
Ability to work effectively with diverse individuals and groups.
Self-starter that is detail-oriented and thinks creatively and strategically.
Ability to produce high-quality work independently.
Demonstrated commitment to social justice, community empowerment, and advocacy.
Qualifications Bachelor's degree in a human services field required.
Or 2+ years\' supervised experience in the mental health field required.
Additional Requirements Satisfactorily pass all relevant screenings and criminal background checks.
Transportation / Travel Driving for work required with transportation of clients preferred in a personally owned vehicle. Travel between sites not required.
Grant / Funder / Program Requirements The Respite Care Coordinator will coordinate and implement services for Respite Program children, youth, and families.
Receive referrals / intake for service and conduct initial intake appointments within the time frame established by the funder.
Coordinate aspects of one-to-one respite, including assignment of staff, attending the initial meeting with clinician, respite staff, and family, and act as liaison between referring program staff and respite workers.
Attend and assist in the planning and execution of group services, including monthly outings, summer program and virtual group.
Manage all aspects of private pay respite including contacting families monthly, arranging payments, and tracking clients and funds.
Ensure completion of all necessary paperwork including assessment, ITP, service notes and program specific forms.
Assist in training/on-boarding of new staff and development of training materials.
Assist with administrative tasks supporting program operations, including quarterly reports.
Carry a caseload of respite clients.
Assist in the development and monitoring of policies and procedures for the respite program.
Stay informed of clients\' service needs to contribute to annual and long-range planning.
Collaborate with other program staff to achieve shared goals.
May serve on agency-wide committees and represent the agency at regional meetings; work effectively with diverse staff and service populations.
Five years of experience working with children in a social service or educational setting, including experience facilitating supportive and therapeutic programming to youth and adolescents.
Excellent verbal and written communication skills.
Ability to work collaboratively with agency staff, parents and community partners.
Ability to interact effectively with staff in multiple programs and diverse individuals and groups.
Demonstrated ability to manage multiple tasks in a fast-paced environment.
Proficiency in Windows, Word, Zoom and Outlook; ability to learn agency and funder computer programs.
Available to work a flexible schedule that includes evenings and some weekends.
Physical Demands While performing the essential duties of this job, the employee may sit and stand for extended periods, talk, hear, and use hands to handle and operate objects. Visual acuity is required for data preparation, transcription, and computer use.
