Position Summary
Residential Care Managers support residents throughout their journey, from admission through transition into housing, ensuring personalized care plans, coordinating resources, and fostering a supportive environment within our two‑step residential program model.
Position Schedule
Full‑time Monday‑Friday business hours (40 hrs. per week) with occasional extended or weekend shifts (minimum of 3 times per year).
Responsibilities
Provide individualized case management for pregnant, postpartum, and new‑mother populations, including newborns to age 5.
Meet residents one‑on‑one to set goals, develop care plans, and link them to housing, employment, education, and wrap‑around services.
Collaborate with other Care Managers to lead weekly case‑coordination group sessions and ensure program accountability.
Facilitate Critical Time Intervention case‑management for residents in their first year post‑graduation, including home visits.
Listen attentively to resident concerns, advocate for their needs, and coordinate with supervisors to develop solutions.
Maintain accurate documentation in case‑management databases and systems.
Advocate on behalf of residents and alumnae for supportive services and decision‑making.
Coordinate crisis interventions and mental‑health, substance‑abuse, and medical referrals; provide transportation as needed.
Work with Residential Leadership on quarterly resource sessions for program graduates.
Provide financial assistance and budgeting support.
Promote personal growth, independence, and accountability among residents.
Collaborate with the Residential Life Team on classes, workshops, groups, and events.

Education and Experience
Bachelor of Social Work with 2+ years relevant case‑management experience; MSW or equivalent considered.
Experience with residential care for high‑risk populations, including women’s homelessness, mental illness, trauma, substance abuse, and poverty.
Knowledge of LA County agencies and supportive housing models.
Christian faith and servant‑leadership orientation preferred.
Other Requirements
Valid California driver’s license, reliable personal transportation, and clean driving record.
