Resident Care Coordinator – Senior Living

Savoy Life’s model of care aims to support senior living communities and help tie residents, their caregivers, and the providers within the community together. We partner with senior living communities to coordinate and manage clinical gaps for residents to improve these community’s core business drivers. We deploy our own team of clinical experts both in-person and virtually paired with technology to provide instant leverage and relief for the community’s staff and fill gaps in care not always met by existing providers in the community.
Position Overview: Savoy Life is seeking a dedicated Resident Care Coordinator (RCC) to provide clinical and operations support within senior living communities in and around Boise, ID. The Resident Care Coordinator will work closely with an assigned Savoy Life RN as well as on-site senior living community teams to improve resident outcomes as well as a virtual provider.
Key Responsibilities:
Care Delivery Back Office Support: Schedule patient visits in the EHR, answer calls, texts, voicemails, manage faxes, enter data, and identify and manage health plan insurance for residents.
Care Coordination Support: Coordinate patient care with the RN and APP, collaborate with community resources, manage telehealth visits, support RCC‑led resident visits, and foster positive relationships with patients, caregivers, families, primary care providers, specialists, pharmacies, and senior living community staff.
Qualifications:
High School Diploma / GED (or higher)
2+ years of experience in healthcare with a focus on care coordination and healthcare administrative support
Medical Assistant, Patient Care Technician, or equivalent certification preferred; otherwise 5+ years of healthcare experience
Experience in geriatric care or senior living community is highly desirable
Must possess a valid and current driver's license
Excellent EMR documentation skills and general comfort with various technologies
Ability to juggle many tasks and switch between visiting residents and coordinating behind the scenes

Hourly Rate: $23-$28/hour
