Job description Under the direction of the Administrator, the Resident Care Coordinator is delegated the responsibility for carrying out the assigned duties and responsibilities of this position in accordance with current existing federal and state regulations and established company policies and procedures.
ESSENTIAL JOB FUNCTIONS Assures appropriate individualized care planning for residents
Provides input/contribution to other multidisciplinary care plans for the residents
Monitors the residents’ behaviors on a daily basis
Implements behavior interventions with residents as needed
Assists other staff with appropriate behavior interventions i.e., making suggestions on approach, coaching staff on implementing interventions, observes staff and resident interactions.
Coordinates activities with other departments as necessary
Provides small group and/or individual activities to meet the resident’s needs and prevent behavior
Completes as needed and in coordination with social services, resident’s assessments related to psychosocial needs
Meets with and works with resident’s facility or responsible party to enhance functioning of residents
Provides in-service to other staff as needed.
Maintains a progress note on the resident’s medical chart on a timely basis and, at least quarterly, completes a progressive assessment note.
Reassesses residents as needed, and every 30, 60, 90 days thereafter, alter care plans and/or initiate transfer or discharge
Provides input/contribution to other multidisciplinary care plans for the residents
Other duties assigned by Administrator
KNOWLEDGE/SKILLS & ABILITIES Must be able to relate and work with ill, disabled, elderly, emotionally upset and at times hostile residents within the facility
Must be able to read, write, speak and understand the English language
Responds to the needs of the facility, residents, and families in a timely and appropriate manner
Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents
Must have patience, tact, cheerful disposition, and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning
Interacts with resident and family members, co-workers, clinical and ancillary staff in nonjudgmental, supportive and calm manner
Maintains confidentiality and discusses confidential information in appropriate areas only
Addresses residents in a respectful manner and knocks before entering residents’ rooms
Knowledgeable of residents’ rights and responsibilities
Requirements RN or LPN in good standing in the state of Illinois
