Position Summary:
Responsible for coordinating and monitoring the assessment and care planning process for residents to ensure quality care and appropriate reimbursement, in accordance with laws, regulations, and facility guidelines.
Responsibilities:
Follow residents through the continuum of care, monitoring resource utilization, service delivery, and compliance with laws and regulations.
Provide ongoing support through assessments, MDS completion, care plans, and evaluations of resident needs.
Enhance resident care quality and satisfaction through case management, utilization reviews, and discharge planning.
Serve as a point of contact for outside managed care contracts, including communication and negotiation.
Maintain regular, punctual attendance per Orlando Health policies and applicable laws.
Ensure compliance with all policies and procedures.
Qualifications:
Graduate of an approved LPN program.
Current Florida LPN license, renewed every two years.
Current BLS certification.
Two years of LPN experience in a healthcare setting.
Knowledge of the MDS process.
