Job Summary
The Nurse Case Manager demonstrates a high level of expertise in the assessment, diagnosis, and treatment of the complex responses of individuals, families, or communities to actual or potential health problems; prevention of illness and injury; maintenance of wellness; and provision of comfort.
Licensure & Registration Requirements for Hire and Continued Employment
Current RN licensure in Virginia or eligible/compact state
AHA BLS HCP Certification or equivalent (for continued employment)
Experience Requirements
Required: Three (3) years of experience including two (2) in a specialty area
Preferred: Two (2) years case management/care management experience
Education Requirements
Required: Baccalaureate Degree in Nursing from an accredited School of Nursing
Preferred: Certification in specialty area as appropriate
Independent Actions & Responsibilities
Makes appropriate decisions and initiates appropriate interventions in the provisions of care coordination within the boundaries of professional practice, the Standards of Practice for Case Management (CMSA) and established policies.
Organizes and plans work with input from the client/family with specific outcomes and demonstrated use of sound clinical judgment and resources.

Determines the appropriate place of contact/intervention with customer including client’s home, community agency, provider office, etc. to achieve customer satisfaction and a positive outcome.
