Schedule: PRN - Dayshift with Occasional Weekends.
Responsibilities Coordinates patient care to ensure continuity, optimal outcomes, satisfaction, and cost-effective treatment.
Collaborates with patients, families, and the healthcare team to support effective discharge planning and resource referrals.
Addresses concerns from patients, families, and payors while monitoring satisfaction and care quality.
Qualifications & Experience Current Virginia state or compact RN license (required).
Basic Life Support (BLS) certification within 7 days of hire.
Bachelor's degree in nursing (BSN) preferred.
3 years of nursing experience required.
Experience in Utilization Review, Case Management, Discharge Planning, and Clinical Leadership is highly preferred.
