Overview Registered Nurse (RN) Case Manager role at St. Croix Hospice. The RN Case Manager plans, organizes and directs hospice care with emphasis on community health education/experience. The professional nurse builds from community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.
Responsibilities Patient Care Complete initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs, including a complete physical assessment and history of current and previous illness.
Provide professional nursing care using all elements of the nursing process.
Assess and evaluate patient status; write and initiate plan of care; regularly re-evaluate patient and family/caregiver needs; revise plan of care as necessary.
Initiate and revise the plan of care as patient status changes; use health assessment data to determine nursing diagnosis.
Develop care plans with goals based on nursing diagnosis, incorporating palliative nursing actions; involve patient and family in planning.
Initiate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed by the physician.
Counsel patient and family in meeting nursing and related needs; provide health care instructions per assessment and plan.
Assist the patient with activities of daily living and support self-sufficiency and comfort care as appropriate.
Act as RN Case Manager when assigned by Clinical Supervisor and coordinate care for the assigned caseload.

Communication Complete, maintain, and submit accurate clinical notes; record pain/symptom management changes and outcomes.
Communicate with the physician regarding needs and changes in condition; obtain/receive physician orders as required.
Coordinate care with community health professionals; teach patient and family self-care techniques; provide medication, diet, and other instructions as ordered; work with the interdisciplinary team.
Maintain a safe patient environment and ensure continuity of care in collaboration with the family/caregiver and hospice interdisciplinary group.
Attend interdisciplinary group meetings.

Additional Duties Participates in on-call duties as defined by policy.
Ensures arrangements for equipment and services are available; supervises ancillary personnel and delegates responsibilities when required.
Maintains and upgrades professional knowledge through continuing education and in-service classes.
Fulfills assigned case duties and participates in quality assessment and performance improvement activities.

Qualifications Graduate of an accredited school of nursing. One to two years of recent acute care experience in an institutional setting, and one to two years of recent home care experience preferred.
Current licensure in the applicable state.
Supervising hospice aides; management experience not required.
Excellent observation, communication, problem-solving, and basic math skills; nursing skills per competency checklist.
Physical requirements: ability to stand/walk for extended periods, lift/transfer patients and equipment; visual and hearing acuity as required for nursing skills.
Must have a licensed driver with a vehicle insured as required and in good working order.
