Primary Function / General Purpose of Position
The RN Care Manager is responsible for coordinating care for patients to support safe, seamless, and timely transitions across the continuum. The role utilizes a collaborative process to identify, assess, plan, implement, and evaluate services necessary to meet an individual’s health–related needs, including social determinants. The RN Care Manager screens, identifies, and assesses individuals in need of active case‑management services and promotes appropriate resources at the right time and place.
Essential Job Functions
Identify and prioritize patients needing care management services, using a holistic approach inclusive of biopsychosocial, functional, cultural, spiritual, and financial factors. Plan with the patient, caregivers, and healthcare team to maximize outcomes.
Complete all required documentation, maintaining clear, concise, and timely records. Ensure handovers at level‑of‑care changes and transitions.
Document plans of care for post‑hospital transitions, ensuring patient/caregiver involvement and preference incorporation.
Follow standardized practices for Advance Care Planning, Length of Stay management, and readmission prevention.
Support denial prevention related to medical necessity by removing barriers and participating in Interdisciplinary Discharge Rounds.
Promote proactive care, address testing and treatment barriers, and offer alternatives to acute care.
Collaborate with revenue cycle partners to ensure appropriate patient classification.
Work with patient access to deliver regulatory letters promptly.
Participate in departmental clinical outcome projects and process‑improvement initiatives.
Maintain respectful, clear communication to achieve facility and department goals.
Promote teamwork and a team‑based approach.
Ensure compliance with CMS and Conditions of Participation for discharge planning and utilization management.
The RN Care Manager stays abreast of community resources to facilitate safe transitions and remains current on clinical advancements.
Licensing / Certification
BLS Basic Life Support – American Heart Association (required). RN license in the state of employment or covered by compact (required). Preferred certifications: Accredited Case Manager Certification (ACM) from American Case Management Association, Certified Case Manager (CCM) from Commission for Case Manager Certification, or Nursing Case Management board certification from ANCC.
Education
Bachelor of Science in Nursing (preferred; required at RSFH).
Work Experience
1 year of experience in a clinical setting (required). 3 years in an acute‑care clinical setting (preferred).
