Schedule: Full Time

Shift: Monday Friday 8-4:30; weekend commitment

Location: Leavensworth and Douglas Counties Kansas

The Role You'll Contribute

Responsible for coordinating and directing home care for patients based on individual needs. This position independently manages the home health patient population, utilizing advanced assessment, teaching, and decision‑making skills. This role ensures appropriate referrals to other services, regular interdisciplinary conferencing, and completion of necessary documentation. Other responsibilities include educating community members and patients, evaluating the effectiveness of the home care plan, and monitoring patient and family responses to achieve goals and outcomes.

Provides comprehensive assessment, planning, implementation, and evaluation for a caseload of home patients.

Optimizes daily schedules to support productivity, efficiency, and best practice visit utilization.

Assess physical, functional, psychosocial, cultural, cognitive status, and discharge planning needs of home care patients.

Evaluates the home environment for safety, infection control, and community resource needs.

Reviews patient history, physical diagnostics, and laboratory data, and reports abnormal results to the physician.

Formulates patient‑specific care plans with the patient, family, and physician, establishing individualized, realistic, measurable goals.

Minimum Qualifications

1 year relevant clinical RN experience

Active American Heart Association BLS

Registered Nurse State Licensure and/or Compact State Licensure

Valid in state Drivers License with current car insurance

Preferred Qualifications

Bachelor's in nursing

Recent, relevant experience in a Medicare‑certified home health agency as a case‑manager

Home Health Case‑Manager Certification

Certificate for OASIS Specialist - Clinical
