Registered Nurse RN Care Manager
The Home Health Registered Nurse (RN) Care Manager coordinates and directs care for home health patients, ensuring appropriate referrals, interdisciplinary collaboration, and outcome monitoring.
The Role You’ll Contribute
The Home Health RN Care Manager is responsible for independent management of the home health patient population, using advanced assessment, teaching, and decision‑making skills. The RN Care Manager ensures that appropriate referrals are made, interdisciplinary conferences take place regularly, and documentation is completed. The Care Manager evaluates patients for appropriateness of home health, develops the home care plan with the physician, educates patients and families, monitors plan effectiveness, and identifies performance improvement initiatives.
Coordinates and directs care for a caseload of home patients as the primary nurse.
Sets priorities for home care caseload, adapts to changing needs, and optimizes schedule daily.
Assesses physical, functional, psychosocial, spiritual, and environmental status and discharge needs.
Formulates patient‑specific plan of care in collaboration with patient, family, physician, and resources.
Implements plan of care through direct patient care, coordination, delegation, and supervision.
Promotes continuity of care and communicates status to other caregivers.
Minimum Qualifications
Associate’s Degree
1 year relevant clinical RN experience
Registered Nurse – State Licensure (Florida or Compact State Licensure)
Valid driver’s license with current car insurance
Active American Heart Association BLS certification
Preferred Qualifications
Bachelor’s Degree in Nursing
Certified Case Manager (CCM) upon hire
Certificate for OASIS Specialist – Clinical (COS‑C) upon hire

Location: Home Health Fieldwork position doing patient home visits in Volusia County, including North Volusia/Flagler County (Palm Coast to Ormond Beach), East Volusia County (Ormond Beach to S. Daytona), West Volusia County (DeLand/DeBary/Deltona/Orange City), and South Volusia County (S. Daytona to Edgewater).
