The Registered Nurse Primary Care - Care Manager is located at the Veterans Affairs, VHA, Lebanon, PA VA Medical Center. Responsibilities include coordinating visits according to the needs and abilities of the patient, linking patients with services and resources to benefit their care, reviewing lab work, consults, and progress notes, completing a physical assessment, assessing patients to problem solve patient care issues, assisting the clinic as needed, and monitoring the Coumadin clinic. The role also involves applying appropriate nursing standards, completing post-Emergency Department (ED)/Urgent Care and post-discharge phone calls within 2 business days, actively managing a high risk registry, providing face to face follow-up visits with patients who recently visited the ED or were recently discharged, completing all required documentation and clinical reminders, assessing needs of the unscheduled patient visitor to the clinic, identifying patients for and updating team high risk registry, participating in team huddles and interdisciplinary team meetings, attending PACT related meetings, and orienting new staff to the PACT model.
