POSITION HIGHLIGHTS

Coordinate Patient-Centered Care: Facilitate collaboration among patients, families, and healthcare teams to develop and implement individualized care plans, ensuring high-quality outcomes and improved patient experiences.

Manage Transitions and Discharge Planning: Conduct assessments, coordinate care resources, and oversee safe, efficient hospital transitions while maintaining continuity of care and supporting medical management initiatives.

Ensure Compliance and Optimize Reimbursement: Evaluate admissions and ongoing stays using approved criteria, communicate with payers to maximize reimbursement, and adhere to all compliance regulations and hospital policies.

Pay

Experienced RN base rate (1+ years of RN Licensure: Range starts at $41 an hour and increases depending on years of RN licensure

QUALIFICATIONS

Successfully passed Board of Registration in Nursing Examination and is licensed in the Commonwealth of Massachusetts

Current American Heart Association Health Care Provider CPR certification required.

Education: Diploma in Nursing
