Job Summary
The Registered Nurse Case Manager (RN CM) promotes patient‑centered care by coordinating the plan of care for a patient stay, managing length of stay, ensuring appropriate resource utilization, and developing safe, appropriate discharge plans in collaboration with the multidisciplinary team. The RN CM facilitates the progression and transition of care using established criteria and supports quality outcomes and patient throughput while balancing optimal care with resource utilization.

Responsibilities

Coordinate activities that promote quality outcomes and patient throughput.

Manage the length of stay and ensure appropriate resource management.

Develop and implement safe discharge plans in collaboration with the multidisciplinary team.

Facilitate the progression and transition of care using established criteria.

Qualifications

Associate Degree in Nursing or Nursing Diploma (Required).

Bachelor’s Degree in Nursing (Preferred).

2+ years experience in case management OR 3+ years experience in clinical nursing (Required).

InterQual experience (Preferred).

Current RN license in the state(s) of practice (Required).

Certification in Case Management (Preferred).
