About the Role: Registered Nurse Case Manager
As a Registered Nurse Case Manager, you’ll play a key role in supporting the health, happiness, and daily experience of the residents at Wesley at Home.
Responsibilities You will have the opportunity to directly and positively impact the health and wellbeing of the people you serve through the provision of professional nursing services.
Support a case load of hospice patients in an assigned area.
Provide compassionate, person-directed care to patients and families.
Open Shifts Full-Time Monday–Friday, Back-up on-call Weekdays and weekends
Qualifications Valid, unrestricted Iowa driver’s license.
Current vehicle insurance.
Current license as a registered nurse in the state of Iowa.
Two years of full-time nursing experience required, with one year of hospice experience preferred.
