What You’ll Do:
Leverage your clinical skills to create, implement, and execute comprehensive care plans aimed at promoting wellness and preventing re-hospitalization for homebound patients.
You will oversee patient care, including teaching patients and caregivers about wound care, blood draws, tube feedings, Tracheostomy care, Foley catheters, CHF management, and medication education. You may also be involved in:
