Job Summary
The RN Case Manager is responsible for facilitating care across the continuum through effective resource coordination, helping patients achieve optimal health, access to care, and appropriate utilization of resources while respecting patient autonomy. The role ensures care is provided at the appropriate level based on medical necessity and assesses transition needs to promote timely throughput, safe discharge, and prevent avoidable readmissions.
Responsibilities
Participate in collaborative programs reviewing patient activities related to medical necessity, length of stay, utilization of services, and post‑discharge needs.
Promote quality care and cost‑effective outcomes to enhance physical, psychosocial and vocational health.
Review, plan, implement, coordinate and evaluate health‑related service options throughout the continuum.
Provide care based on physical, psychological, educational, safety and related criteria for neonate to geriatric populations.
Qualifications
Associate Degree in Nursing (required); Bachelor’s Degree in Nursing (preferred).
Case Management certification (preferred).
Current Texas RN licensure with the State Board of Nurse Examiners.
1+ years of experience in healthcare.
