Registered Nurse – Case Management
St. David’s South Austin Medical Center, part of HCA Healthcare, is seeking a dedicated Registered Nurse Case Manager to coordinate high‑quality patient‑centered care across inpatient and observation settings.

Job Summary

Promote patient‑centered care by coordinating the plan of care across the patient’s stay.

Manage length of stay, ensure appropriate resource utilization, and develop a safe discharge plan in collaboration with the multidisciplinary team.

Facilitate the progression and transition of care using established criteria and support optimal resource utilization.

Key Responsibilities

Provide case management services for assigned inpatient and observation patients.

Identify patients at risk for adverse outcomes during the transition of care.

Perform comprehensive psychosocial, medical, and discharge needs assessment.

Reassess clinical condition as indicated and develop strategies to mitigate readmission risk.

Coordinate plan of care and lead discharge planning with the multidisciplinary team.

Partner with Social Services to ensure appropriate post‑acute medical needs.

Qualifications

2+ years experience in case management OR 3+ years clinical nursing.

Associate Degree in Nursing or Nursing Diploma (Bachelor’s preferred).

Current RN license in the state of practice.

InterQual experience preferred.

Apply for a Texas RN License within 60 days of hire; obtain within 6 months.
