Essential Job Functions (EJFs):
Attends work on a regular basis and may be required to work a specific shift schedule or rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.
Monitors the health status of all individuals assigned to the RN Case Manager. Conducts rounds, observes, and interacts with individuals, oversees nursing interventions, and evaluates outcomes of care.
Provides hands-on nursing care as needed and guides LVNs and direct care staff to implement health care plans.
Participates as a member of the interdisciplinary team (IDT) to coordinate health care, educate team members about care plans, and advocate for the needs and preferences of the individual.
Prepares for essential meetings related to individuals on the caseload and updates necessary items in automated systems and tracking documents.
Performs physical assessments on a monthly, quarterly, and annual basis and as needed to develop care plans and referrals.
Completes required documentation on a schedule and meets deadlines for plans of care, assessments, and other records.
Reviews physician orders and MAR to ensure medications are administered as ordered and analyzes data for trends to guide actions.
Attends on-campus medical appointments and coordinates off-campus specialist appointments when required.
Participates in competency-based training, maintains nursing licensure, follows safety and infection control practices, and adheres to information security protocols.
Establishes professional relationships, mentors new RN Case Managers, and participates in quality assurance reviews and committees.
Provides treatment to HHSC employees injured on the job as needed and supports disaster response and COOP activities as required.
Knowledge, Skills and Abilities (KSAs):
Knowledge of nursing techniques, standards, procedures, medical regulations, and health care principles.
Skill in patient care and in using computer software and electronic health records.
Knowledge of state and federal laws and IDD-related regulations affecting State Supported Living Centers.
Ability to interact effectively with individuals with IDD, families, and staff; interpret policies; and recognize patterns of medical necessity and treatment.
Ability to perform comprehensive physical assessments, interpret data, and develop plans of care; strong time management and prioritization.
Excellent verbal and written communication skills; ability to work independently with all levels of staff.
Maintain current CPR certification and knowledge of emergency procedures and equipment.
Ability to interpret regulations and communicate them clearly; ability to assess and respond to emergencies.
Registrations, Licensure Requirements or Certifications:
Licensed to practice as a registered nurse in the State of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact.
Initial Screening Criteria:
At least one (1) year experience as a practicing registered nurse is required. Experience in assessment and documentation of health status is required. At least two (2) years of experience in a health agency, hospital, or health-care facility is preferred. A Bachelor\'s Degree is preferred. Previous experience with Individuals with Intellectual and/or Developmental Disabilities (IDD) is preferred.
Additional Information:
Applicants must pass fingerprint background check, pre-employment drug screen, and registry checks including CANRS, Nurse Aide, Medication Aide, and LEIE.
