Role you’ll contribute
The RN Care Manager collaborates with the patient, family, social workers, nurses, physicians, and the interdisciplinary team to ensure patient‑centered care coordination and progression through the continuum of care. The RN Care Manager:

Ensures efficient and cost‑effective care through appropriate resources monitoring and clinical care escalations.

Works under the general supervision of the Care Management Supervisor, Manager, or Director of Nursing.

Conducts patient evaluations of post‑hospital needs, develops transition‑of‑care plans, and initiates their implementation prior to discharge.

Enhances optimal patient flow/throughput to improve continuity of care, smooth and safe transitions, patient satisfaction, patient safety, readmission prevention, and length‑of‑stay management.

Communicates daily with the interdisciplinary team during daily multidisciplinary rounds.

Provides education to nurses, physicians, and the interdisciplinary team on utilization of resources, medical necessity, CMS CoP for discharge planning, and care coordination.

Keeps current knowledge of post‑hospital care services such as Home Health, Infusion Services, Durable Medical Equipment, Palliative Care, Hospice, Outpatient Services, Transitions of Care Clinics, and community‑based organizations.

Adheres to departmental and system goals, objectives, policies, and procedures, ensuring quality patient care and regulatory compliance.

Actively participates in outstanding customer service, maintaining respectful relationships with all.

Completes initial evaluation for transition‑of‑care needs on all identified patients within one calendar day of admission.

Interviews patients and involved caregivers, reviews current and past inpatient and outpatient medical records.

Reviews labs, medications, History and Physical, therapy notes, ED notes, test results, and progress notes.

Incorporates patient/family care goals and preferences into the transition‑of‑care plan and communicates these to the multidisciplinary team.

Integrates clinical, social, and financial factors into the transition‑of‑care plan.

Meets with patients/families to discuss realistic and appropriate discharge options and post‑hospital care providers.

Qualifications

Associate’s Degree in Nursing or RN Diploma.

Registered Nurse (RN) license.

Minimum of 2 years of medical/hospital nursing experience.
