Position Overview

The Care Manager is responsible for providing care management to a group of health plan members, creating the Person-Centered Service Plan (PCSP), coordinating services, and documenting care in accordance with health plan standards and under the guidance of the Care Management Supervisor.
Performance Responsibilities and Standards

Provides care management for members through an interdisciplinary approach involving assessment, planning, facilitation, and advocacy to support member needs.
Implements a quality, cost-effective plan of care utilizing all available resources, in collaboration with the member, family, healthcare team, providers, and community programs based on the UAS-NY assessment.
Oversees caseload of managed care members.
Performs all activities in compliance with regulatory requirements; completes clear, concise, timely documentation. Coordinates support staff including LPNs, Community Health Workers, and Care Coordinators, providing guidance and education for efficient patient management.
Participates in departmental and staff meetings, and training as required.
Adheres to privacy regulations including HIPAA and organizational policies regarding confidentiality and security.
Communicates effectively with members, families, staff, and providers; acts as a liaison and advocate.
Demonstrates cultural competence respecting members' beliefs, language, and support systems.
Performs other duties as assigned.
Requirements

Registered Nurse license and current registration in New York State; BSN preferred, associate degree required.
Minimum one year of nursing experience.
Experience in long-term or managed care is a plus.
Knowledge of home care, Medicare, Medicaid is a plus.
Care Management Certification is a plus.
Bilingual skills are a plus.
Ability to work independently, prioritize, and utilize computer and Microsoft Office skills.
Strong communication skills, both in person and virtually.
Valid NYS driver’s license and ability to travel in all weather conditions.
Physical Requirements

The role is remote with required home visits. In-office attendance may be needed. Standard postures include standing, sitting, bending, walking, and driving. Frequently uses a laptop. Sensory requirements include speech, vision, smell, hearing. Lifting up to 20 pounds frequently, up to 50 pounds occasionally. Exposure to bloodborne pathogens.
Compensation & Benefits

Salary: $72,415.71 - $83,922.76 annually with a $15,000 sign-on bonus (restrictions may apply). Includes retention bonuses, 401k with match, health plans, tuition reimbursement, work/life balance, HSA, recognition programs, PTO, holidays, sick leave, disability, EAP, and more.
